SUBJECT:  Sliding Fee Discount Program for the McKenzie River Clinic
EFFECTIVE DATE:  January 2017
POLICY: A sliding fee discount, based on Federal poverty guidelines, will be made available to qualifying patients.
PURPOSE: This program is designed to provide free or discounted care to those who have no means, or
limited means, to pay for their medical services (Uninsured or Underinsured). In addition to quality healthcare, patients are entitled to financial counseling by someone who can understand and offer possible solutions for those who cannot pay in full. We will assist the patient in contacting the Oregon Health Plan patient advocate who will work with the patient and/or guarantor to find reasonable payment alternatives.
The McKenzie Clinic will offer a Sliding Fee Discount Program to all who are unable to pay for their services. The Clinic will base program eligibility on a person’s ability to pay and will not discriminate based on age, gender, race, sexual orientation, creed, religion, disability, or national origin. The Federal Poverty Guidelines,, are used in creating and annually updating the sliding fee schedule to determine eligibility.
PROCEDURE: The following guidelines are to be followed in providing the Sliding Fee Discount Program
Notification: The McKenzie River Clinic will notify patients of the Sliding Fee Discount Program by:
- Payment Policy Brochure will be available to all uninsured patients at the time of service
- Notification of the Sliding Fee Discount Program will be offered to each patient upon admission
- An explanation of our Sliding Fee Discount Program and our application form are available on the Clinic website (
- Notification of Sliding Fee Discount Program can be seen in the clinic waiting area
All patients seeking healthcare services at the McKenzie River Clinic are assured that they will be served regardless of ability to pay. No one is refused service because of lack of financial means to pay.

Request for discount: Requests for discounted services may be made by patients, family members, care coordinators or others who are aware of existing financial hardship. The Sliding Fee Discount Program will only be made available for clinic visits. Information and forms can be obtained from the Front Desk and the Clinic Administrator.
Administration: The Sliding Fee Discount Program procedure will be administered through the Clinic Administrator’s office or designee. Information about the Sliding Fee Discount Program policy and procedure will be provided and assistance offered for completion of the application. Dignity and confidentiality will be respected for all who seek and/or are provided charitable services. 
Alternative payment sources: All alternative payment resources must be exhausted, including all third-party payment from insurance(s), Federal and State programs.


​​As a patient at the McKenzie River Clinic you have the responsibility to :

  • arrive on time for your appointment. If you are more than 10 minutes late your appointment will need to be rescheduled.
  • notify the Clinic at least 24 hours before your scheduled appointment if you are unable to keep the appointment. 
  • call your pharmacy for prescription refills at least 3 days before the refill is needed.  Do not call the Clinic for refills even if the bottle says "no refills."  The pharmacy will check with the Clinic if necessary. 
  • provide the Clinic with any changes in your phone number or address, so that we always have a way to contact you with information about your care.
  • request that your previous primary care doctor and any specialists  you might have seen send your medical records to the Clinic.
  • provide the Clinic with a current copy of your Advance Directive.  If you do not have one, ask your Nurse Practitioner about it. 
  • ask questions about anything you don't understand, and don't be hesitant to bring up any concerns about your health and care.

  • be treated with consideration, respect and dignity without discrimination because of race, creed, color, national origin, ancestry, religion, gender, gender identification, sexual orientation, marital status, age, or handicap.
  • ​receive evaluation and management of pain.
  • have all communications and records pertaining to your health care kept confidential.
  • receive an explanation of your bill regardless of the source of payment.
  • receive considerate and respectful care in a safe and private environment.
  • be an active participant in the planning of your care.
